Attending Physician
The attending physician evaluates and manages the patient’s medical status and treatment plan on an ongoing basis. This ensures that the proper care is given and that your loved one’s health is continuously considered and that treatment is adjusted as needed.
Medical Director
At Santé, our expert medical director oversees the physician team, providing expert hospice perspective and guidance.
Registered Nurse
Santé’s registered nurse visits each patient at bedside, whether they receive hospice services at home or in a comfortable inpatient setting. The registered nurse closely monitors your loved one’s condition, administers treatments and medications, and facilitates communication between all members of the care team.
Clinical Director
The clinical director, who is also a registered nurse, is responsible for managing and coordinating the care between the patient and the interdisciplinary team. His or her role is to ensure there are qualified, compassionate nurses assigned to each case, that each treatment plan is appropriately implemented, and that the patient’s wishes are followed at all times.
Social Worker (MSW)
The social worker’s primary responsibility is to provide emotional support to the patient, his or her family, and caregivers. Social workers are also knowledgeable resources that can connect patients and families with local support organizations.
Certified Nursing Assistants (CNAs)
CNAs play a vital role in each patient’s experience in hospice care. They provide the majority of direct hands on care each day, including assistance with eating, bathing, toileting, dressing, and more. They can provide this assistance at home or in an inpatient facility.
Chaplain
Our non-denominational hospice chaplain can provide spiritual support and communicate with your personal spiritual advisor or clergy member to schedule visits and request resources.
Hospice Volunteers
Finally, Santé’s hospice volunteers are dedicated to ensuring hospice patients have the companionship they need during this stage of life.
